Preparing for Your First Appointment

To help us provide the best care possible, please bring the following documents to your initial appointment:

  • Proof of Insurance (if applicable)
    • Medicaid card (current) and monthly eligibility card
    • Medicare card
    • Veterans Administration (V.A.) outpatient card
    • Group or private insurance card
  • Proof of Income (Required for financial assistance or adjustments to service charges)
    • Previous year’s 1040 tax return
    • Current paycheck or pay stub
    • Social Security benefits award letter
  • Proof of Residence (Required for financial assistance or adjustments to service charges)
    • Utility bill (electric, water, gas, phone)
    • Lease agreement

Additional Information

  • Individuals aged 16 and older may request inpatient or outpatient services without parental consent.
  • If financial circumstances change during treatment, please notify the front desk so adjustments can be made.
  • If someone has Power of Attorney or legal guardianship for the person seeking services, legal documentation must be provided.
  • If a child’s parents are divorced, the custodial parent must provide legal documentation. If a child is adopted, the adoption order must be presented at registration.
