Prefer experience working with persons with mental illness; experience preferred in working with families and utilizing community services. Completion of orientation and new hire training within the first six months of employment.

Case Manager will assist consumers and family in assessing needs, coordinating access to necessary care and services appropriate to their needs, developing treatment goals, utilizes community resources, developing social supports systems, provide skills training, collaborate with MCOT to provide follow up, relapse prevention and crisis assessment and other duties as assigned.

Bachelor's Degree in a Human Services Field.
